Most Common Cosmetic Dermatologic Procedures

ASDS Consumer Cosmetic Dermatologic Procedure Survey Recently, the American Society of Dermatologic Surgery released their consumer survey of Cosmetic Dermatologic procedures.   There were many interesting findings why patients were seeking such procedures and what procedures they were looking for. Who influenced these consumers regarding their procedures? Surprisingly, while 50% of these consumers listed their Dermatologist as the most important, more than 40% listed their friends and more than 30% listed their primary care physician.  This indicates the importance of referrals from friends and families as well as the importance of other physician specialties in the recommendation of cosmetic procedures. Why are consumers exploring cosmetic dermatologic procedures? These finding were very interesting and also not surprising.  Consumer comments included “I want to feel more confident”, “I want to appear more attractive”, and “I want to look as young as I feel or better than my age”.  These are common refrains that we hear from our patients at Beleza Medspa.  When asked for reasons why they are waiting for such procedures they commented “Cost”, “May be painful” and “May not get the results I am looking for”.  Beleza medspa tries to reduce such concerns by offering specials and deals, less invasive and painful procedures and by trying to offer only procedures that are proven to be clinically effective. Christie Brinkley Named Spokeswoman for Ultherapy

ULTHERAPY: NON-INVASIVE SKIN TIGHTENING As you may have seen in the news last October, Christie Brinkley came forward to People Magazine about her experience with two anti-aging procedures: Ultherapy and Xeomin. While this news may have come as a shock to most people, for me, I was relieved! For years Brinkley, 63, has claimed her anti-aging secrets include a strict vegetarian diet, an intense fitness schedule, and an over-the-top skin care regimen with the occasional filler and laser treatment. I always thought there might be more to the story and I praise her for her honesty. I personally underwent Ultherapy for the first time almost three weeks ago on my crow’s-feet and under my eyes. For years I have battled fine lines under my eyes and they have always bothered me. I have almost tried every noninvasive procedure (except filler) and I am not interested going under the knife at this point. To be clear, I receive regular Botox treatments, I have consistently used of top-of-the-line eye creams and gels since I was 19, I have had Microneedling with PRP, and I also had PRP injections under my eyes last July. I still have fine lines under my eyes. I realize we are all our own worst critics, but everyone’s got their “thing” and my under-eye lines bother me. I am 33 years old now and I know those lines are just going to get worse, so I opted to try Ultherapy, to see what Christie Brinkley has been raving about. And, let’s be honest. I am working for a top-ranked medical spa, there’s no excuse not to try everything, right!? Before I get into my experience with Ultherapy at Beleza, let’s break down the basics of the procedure so everyone is on the same page. WHAT IS ULTHERAPY? Ultherapy is a non-invasive / nonsurgical procedure using ultrasound technology to lift the brows and mid-face and tighten the neck and area under the chin. It also can treat fine lines and wrinkles in the décolletage area. What I like about this innovative technology is that it uses your body’s own regenerative response to gently and gradually lift skin and smooth lines and wrinkles. It is also the only non-invasive treatment cleared by the FDA to actually lift skin — who knew!? HOW DOES ULTHERAPY WORK? Ultherapy uses ultrasound imaging to pass energy through the superficial layers of the skin to target the deeper, core area where collagen production occurs. What is great about Ultherapy being the only cosmetic procedure to use ultrasound imaging is that it allows your provider to see the layers of tissue being targeted to ensure the energy is distributed where it will be most beneficial. During the procedure, your tissue will undergo what is called “neocollagenesis,” which is the increased production of new collagen. Over time, this increase produces a noticeably rejuvenated, uplifted look, which is exactly what you want to happen! DOES ULTHERAPY HURT? This is a hard question to answer. To be perfectly honest, yes, there were parts that were uncomfortable, but looking back at my procedure, it really was not bad at all! To be effective, Ultherapy must heat the deeper layer of tissues under your skin, so it will definitely cause some discomfort in particular areas. For a regular treatment, the provider will typically do two passes over the treatment area, each at a different transducer lever. At an additional charge, you can opt for a third pass over the area, which is what I decided to do since I wanted the best treatment I could get. In a nutshell: The first pass was a cakewalk, the second pass started to get uncomfortable (not unbearable), and by the third pass when my tissues were extremely heated, I definitely felt discomfort, but it was over before I knew it. That said, I would do it again in a heartbeat and truly do not think there is anyone in the world who could not handle this procedure. The good thing is that the discomfort only lasts for a second and then it’s over. ULTHERAPY SIDE EFFECTS After my Ultherapy treatment, I had some redness and mild swelling under my eyes and on my crow’s-feet that lasted only a few hours or so. I actually liked the swelling because it made my cheek bones pop and I even had two people ask me if I had gotten Voluma that day! The following week I noticed I was sensitive under the skin where we did the procedure. Nothing intolerable at all, but definitely noticeable when I would apply my products and sunscreen. Other than that, no side effects or downtime whatsoever. What are Fraxel Laser Treatments? Fraxel is a brand name for a new type of laser technology developed in 2001.  The technology involved the refinement of the CO2 laser.  The Co2 laser had been the gold standard for laser resurfacing treatments.   The light generated by the CO2 laser is highly absorbed by water, which is what skin is mostly made of.  As a result, the CO2 laser would vaporize the outer layer of skin it treated.  This would remove all signs of visible sun damage and aging.  The results were great but the patient was left with an open, burn-type wound to deal with after the procedure.  Healing took weeks and patients were prone to infections and complications.  The procedure required general anesthesia and pain was significant. Fraxel brought a new wrinkle to the CO2 laser.  It fractionated the laser beam, breaking it into millions of tiny beams in a grid.  The point of the Fraxel laser was that these microscopic pulses of light treated small volumes of the skin, leaving untreated areas between them.  The advantages of this approach were many.  By leaving untreated skin between the treatment zone, the outer layer of skin remained intact.  This skin acts as a natural dressing, protecting the underlying structures until the new skin is ready. The untouched cells in the intact skin could also participate in healing.  This decreased pain, downtime and complications. How do Fraxel Laser Treatments Work? The treatment zones of the fraxel laser actually vaporize the skin.  These areas allow the skin to refill them with new collagen, the building block of the skin.  The areas surrounding the treatment zones absorbs a lot of heat too.  This causes their collagen to eventually  be rebuilt and tighten.  The results are a new outer layer of skin within 3-4 days of the treatment.  The new skin is a shiny, pink that eventually resolves into a smooth surface.  The even more exciting part of the Fraxel laser is the skin tightening.  The fractionated laser beams penetrate deep into the lower layer of the skin, the dermis.  This is where the collagen is made and the heat stimulates new collagen and tightening which starts 3-4 weeks after the procedure. What is an ablative laser? An ablative laser remove skin during the treatment.  The skin is usually vaporized into smoke.  This allows for removal of sun damage and signs of aging and maximal results.  A nonablative laser does not remove skin.  The heat is delivered into the skin to cause coagulation of the collagen as well as stimulation of new collagen.   A new skin surface is not produced, but skin tightening is. What is Collagen Protein & Collagen Peptides Found in the connective tissue of animals, especially vertebrates, collagen is a group of proteins that  are naturally found in the body. Collagen protein serves multiple functions for humans, including being the basis for connective and fibrous tissues. At Beleza, most of the focus on collagen is centered on its’ role in skin elasticity and firmness. Collagen serves as the structural support for skin, so as we age, this structure decreases. With time, age and collagen breakdown, skin looses firmness and the ability to bounce back when pulled or stretched. What is shrink wrap liposuction? There has been a lot of buzz and interest lately in a new procedure called shrink wrap liposuction.   Unfortunately, shrink wrap lipo is just another name for laser liposuction or smartlipo.  A physician in California has just marketed the procedure under a new name.  Although the name is catchy and clever, the procedure offers nothing new over conventional laser liposuction. The good news is that laser liposuction works and is here to stay.  Initially written off by the plastic surgeons as an unnecessary procedure, laser liposuction has progressed to almost be standard of care during liposuction these days.  Smartlipo, the original laser liposuction system has been joined by many competitors like Slim Lipo and Cool Lipo.  The power of the lasers have increased as well the different wavelengths of light used.  There are as many choices for these procedures for the surgeons as well as the patients. What is the advantage of laser liposuction? Laser liposuction is a tool and not a replacement for a skilled surgeon.  It is just another instrument available to the surgeon to improve your results.  What laser liposuction offers is the ability to introduce heat into the fat and skin layer.  The heat is proven to have several critical effects.  Fat cells absorb the heat and literally melt.  So the laser can help remove more fat and enable finer body sculpting results.  The other important effect is skin tightening.  Heat is well known to cause collagen stimulation and collagen contraction. Collagen is the building block of your skin and connective tissue.  So applying heat to your skin and deeper connective tissues leads to skin tightening (shrink wrap effect). Laser liposuction has been compared to traditional liposuction as a better alternative.  However, laser liposuction still involves traditional liposuction.  When a liposuction case begins, there is usually a large fat layer that must be made smaller.  If the layer is very small, the laser can be used first to melt the fat prior to suctioning it away.  In most patients, the layer is too large and the small laser fiber would be inefficient in trying to melt this thick fat.  It is best to suction away most of the fat with traditional as well as assisted liposuction techniques like vaser and power-assisted, prior to using the laser.  Once the fat layer is at the desired thickness, the laser can then be brought in to fine tune and smooth the fat layer, perform body sculpting and induce skin tightening. Laser liposuction does also have several other advantages like decreased bleeding and reduced downtime.  The heat from the laser appears to cauterize some blood vessels and reduce bleeding.  By substituting laser liposuction for the some of the more traumatic traditional lipo techniques, tissue damage and downtime also appear to be reduced. When looking for a laser liposuction surgeon it is crucial to make your decision on their experience and results rather than on their technology.  Repeated studies have shown that the power or the wavelength of the laser have no relation to good results.  Remember, laser liposuction is just another tool and its how the surgeon uses it that will determine your outcome. What is Fractional Laser Resurfacing? The  gold standard of laser resurfacing, has been the CO2 laser for years.  Offering great results, the old CO2 laser required general anesthesia and had an extended recovery.  The problem with the procedure was that the laser would remove all the surface skin.  This was very effective at erasing signs of aging, but left the patient with an open burn wound to deal with.  This led to more than 2 weeks of difficult downtime.  The advance of the Fractional laser resurfacing was to not remove all the skin.  A fractional pattern to the laser beam allowed only some of the skin to be removed while leaving untreated areas between the treatment zones.  The same amount of energy as the old CO2 was delivered, but the old skin remained intact.  This old skin acts as a natural band-aid to protect the new skin until it is ready.  There are no open wounds and recovery is much easier and quicker. The old skin containing the sun damage peels in 3-4 days, and a fresh, pink, shiny new skin is reveled.  There will be some redness for another 3-4 days that gradually subsides.  The exciting thing is that this is just the beginning of the results.  The resurfacing does cause a dramatic change in appearance alone.  However we also introduce a great deal of heat to the lower layer of skin where the collagen is made.  Around 3-4 weeks after the procedure, new collagen will begin to be made and lead to skin tightening.  So the fractional Laser CO2 offers both resurfacing and skin tightening in one procedure, with much less downtime. Skin Tightening with Venus Freeze, Thermage and Smartlipo Laser Liposuction. Everyday there appears to be a new miracle skin tightening and fat reducing device on the market.  Most of them make incredible claims with little to no peer reviewed research to back them up.  It is confusing not only for the consumer but for the doctor too.  So how do you decide which treatment is most effective and affordable?  Almost all skin tightening technologies utilize heat to accomplish this task.  Collagen is a protein that is the building block of skin.  It is manufactured in the deeper layer of skin, the dermis.  It has been known for a long time, that heating this layer stimulates the skin cells to make more collagen. Furthermore, heating the collagen itself causes some shrinkage of the collagen bundles.
