Breast Lift SurgeryWhen addressing breast concerns I feel there are two keys  question that must be answered up front.1. are you happy with the breast volume?2 are you happy with the breast position/shape?These are two very different issues and should be addressed separately.  For example – I have seen many women over the years who wanted a lifted breast and instead received a large implant – yes the implant will fill out the extra skin – however if extra volume is the primary desire she will not be happy with the outcome.  It is important to understand what a breast augmentation and/or a breast lift can do for you.Breast AugmentationWhen addressing volume we have techniques to both reduce the amount (reduction) and well as augmentation with Breast Implants or Fat Transfer Breast Augmentation.Breast Lift SurgeryOnly after the volume issue is addressed can you address the position of the breast, which is just as important.  Ptosis,  or sagging of the breast, must be addressed if an implant is being placed.  Excess skin could allow the breast to hang off the implant creating a significant deformity.  Breast lift procedures can be done with or without volume augmentation.  Breast lift with an implant will address the excess skin only.  Breast lift without an implant will first rearrange the breast architecture (creating a more round and protruding breast) then will remove the excess skin.Breast Reduction SurgeryBreast reductions are typically performed for one of two reasons: A perceived abundance of breast tissue with a cosmetic desire for a smaller breast  or for improvement in quality of life. What’s a Mommy Makeover?

Mommy Makeover Surgery: Staci Hix-Hernandez MD Approach. >Traditionally a mommy makeover is thought to be a tummy tuck with a breast augmentation +/- lift.  however, no two women are alike, and certainly no two pregnancies are alike.  Some women gain more weight and lose more weight than others.  Some women are blessed with great genetics and end up with no body changes after pregnancy.  However, many women suffer loose skin, relaxed muscles and stretch marks.  There is a lot to say about going into pregnancy as healthy as you can as a way to avoid these problems.  But once you have them, what can you do about them?What’s a Mommy Makeover?>I view a mommy makeover as addressing the concerns that arose as a result of pregnancies.  For each individual this will be different.  There are certain anatomical changes that occur as a result of pregnancy that diet and exercise alone cannot fix.  Its not always the breast and stomach.  Maybe the primary concern is her neck or arms, perhaps the thighs.>The most important thing is to sit and listen.  I need to discover what each patient is concerned about as well as what her outcome goals are. The breasts can take a beating after pregnancy and breastfeeding.  Many women are left with deflated and droopy breasts.  The approach to breast rejuvenation in the mommy makeover involves a breast lift or mastopexy and/or breast augmentation.  Some degree of sagging breast tissue may be dealt with breast implants alone, but significant breast sagging will most likely require a breast lift.  If the breasts are deflated, they can be augmented with breast implants or natural breast augmentation with fat transfer. Tummy Tuck The abdomen takes the brunt of the effects of pregnancy.  It stretches out to accommodate the pregnancy and then is expected to return to normal.  Unfortunately, sometimes the muscles of the abdominal wall come apart, letting the abdomen to bulge out.  This is where a tummy tuck or abdominoplasty comes in.  These muscles are actually sewed back together leaving a tight, flat belly.  The abdominal skin can also be stretched out leaving sagging and stretch marks.  The tummy tuck addresses this too, removing the excess skin and stretch marks. Liposuction vs Tummy Tuck.This is an age-old plastic surgery question that both patients & surgeons have struggled with.  In except the most obvious cases, there is no one right answer for everyone.  There are many individualized factors that come in to play. Both liposuction and tummy tucks have their advantages and certain downsides.What is Liposuction? Simply put, liposuction is the suctioning of fat from the body.  Now it’s more complicated than that, as liposuction technique approaches an art form.  Removing fat in the right amount from the right areas to achieve body contouring, requires an artistic eye.   So liposuction is ideally suited to remove fatty deposits from around the body. The other lesser known effect of liposuction is skin tightening.  We routinely see at least 50% skin tightening after a liposuction procedure.  This is important, as the fat layer gets smaller we want the skin to follow along.  This is the basis of shrink-wrap liposuction.  There are several advanced liposuction techniques to try to achieve even more skin tightening.  They involve performing superficial liposuction, right under the skin to maximize the post-procedure skin retraction.  Laser Liposuction using Smartlipo can also be used to introduce heat which further tightens the skin. The best thing about liposuction is that it can be safely performed under local anesthesia.  There are no big incisions, just multiple small entrance points.  The patient can resume normal activity almost immediately and downtime is minimal.What is a Tummy Tuck? A tummy tuck (abdominoplasty) is a surgical procedure mainly indicated to deal with loose skin and muscles of the abdomen.  In many women, the weight gain during pregnancy stretches out both the abdominal wall and skin.  This leads to droopy, protuberant belly.  The tummy tuck deals with this by removing the loose skin and tightening the abdominal wall.   The tummy tuck procedure requires a rather large incision below the bikini area.   The loose skin is then pulled down and the excess removed.  Usually this requires the belly button to be repositioned.  Deep stitches are then placed to sew the loose abdominal muscles together.   Liposuction can be combined with a tummy tuck for body contouring. The advantages of a tummy tuck is that it will leave you immediately with a flat abdomen, remove stretch marks and tighten your muscles.  It can be performed under local anesthesia, but usually, general anesthesia is required.  The downside includes the large scar, higher rate of complications, more downtime and limitations, a ‘surgical’ looking abdomen and added cost.So Liposuction Vs Tummy Tuck, which is right for me? Tumescent Liposuction describes a type of local anesthesia done while awake. The word tumescent means firm and reflects the feel of the fat layer after anesthesia is infused.  The anesthesia consists of a saline (salt-water) solution that contains lidocaine, epinephrine and bicarbonate.  Lidocaine is a local anesthetic that you commonly get at the dentist.  It blocks the nerve fibers that sense pain.  Epinephrine is a drug that causes blood vessels to constrict.  It is used to prevent bleeding during the procedure as well as prolong the effect of the lidocaine.  Bicarbonate is similar to baking soda and is used to adjust the pH of the fluid to reduce its acidity. The tumescent fluid is mixed prior to the liposuction procedure.  It is infused through small incisions into the fat layer through a hollow tube (cannula).  As the fluid is pumped into the fat layer, it causes it to expand and get firm.  This provides working space for the liposuction procedure and minimizes trauma to important structures.  Commonly a waiting period of 15-30 minutes then follows.  This allows the lidocaine and epinephrine to settle in, numbing the area and constricting the blood vessels. Once the waiting period is over, the liposuction procedure then can begin.  The area to be treated is totally numb by now and the patient may feel some vague sense of movement.    At this point, the tumescent anesthesia has another important function.  The fat is solid and the tumescent liquid provides a way for it to be suctioned from the area.  At the end of tumescent liposuction, the remaining anesthetic  provides continuing numbness as the patient returns home.  The residual tumescent fluid is usually allowed to drain from the liposuction area.  By doing this, healing is sped up and infection prevented as any bacteria will also drain from the area.  Massage, exercise and compression garments are used to help drain the tumescent fluid and resolve the swelling.  Final results can take 3-6 months.Tumescent Liposuction Advantages:provides anesthesia during and after procedure without risk of general anesthesia. important structures retain sensitivity and cannot be harmed without you feeling it. blood loss minimized due to effect of epinephrine. provides working space for liposuction as well as protecting surrounding tissues from trauma. allows solid fat to be suctioned. prevents infection by allowing fluid to drain. virtually no recovery and the postoperative risks of surgery due to bed rest are avoided. no anesthesia fees decreased risk of nausea and vomiting. you are awake!  Click Here for More About Tumescent Liposuction     …
