Eye Wrinkles

What are the types of Eye Wrinkles?Our eyes are constantly opening and closing every time we blink.  Plus, when we smile and make other facial expressions, our eyes have a role too.  All this movement eventually causes wrinkles in the skin around the eyes.  The eye is surrounded by a circular muscle that acts to open and close the eyelid.  The wrinkles that result from its movement, tend to form in lines radiating from the center of the eye.  These eye wrinkles can run up to the eyebrow and down to the cheek.  These under eye wrinkles are better known as Crows Feet.What causes Eye Wrinkles?Did you ever wonder why children don’t have eye wrinkles as they are constantly opening and closing their eyes too?  The answer is contained in the overall condition of the elasticity of the skin.  Children have a lot of elasticity and their skin easily and readily bounces back after movement.  However, as we age, we lose that elasticity and after a movement, the skin remains folded and an under eye wrinkle eventually forms.What causes the loss of skin elasticity?The easy answer is the sun.  Damaging UV rays from the sun, damage the two main components of the skin, collagen and elastin.  These proteins are the building blocks of skin and are critical for healthy, youthful appearing skin.   Sun damage not only damages collagen and elastin in the skin, but also decreases their production.  The result of the loss of collagen and elastin is skin that loses its elasticity and allows eye wrinkles to form.What can we do to treat eye wrinkles?There are several approaches to treat eye wrinkles.  The first involves relaxing the circular eye muscle around the eye that causes them.  Our goal is to relax the muscle’s movement, not paralyze it.  Botox and Xeomin, are well known neuromodulators that can serve this purpose.  A few relatively painless injections of Botox or Xeomin placed around the eye can help to reduce eye wrinkles and more importantly, prevent new eye wrinkles.  Botox and Xeomin last around 3-4 months, and continued maintenance is required.  If the wrinkles are real deep, then a dermal filler like Juvederm may be required.  Fillers like Juvederm can fill in the wrinkles and also replace lost under eye volume.  Botox and Juvederm work the best together to completely erase eye wrinkles.The other treatment for eye wrinkles is to improve the elasticity of the skin around the eye.  The easiest first step is to use an eye cream.  The most effective eye cream is a retinol or retinoid based product.  Retinol and retinoids actually activate the skin cells to produce new collagen and elastin, actually rejuvenating the skin.  There are many other products that are complementary to retinols and retinoids like Vitamin C, peptides, etc.  Of course none of these products are useful if you do not prevent more sun damage with a sunscreen.More advanced cases of sun damage and eye wrinkles, require procedures that can stimulate even more collagen production.  These procedures include chemical peels, microneedling and laser resurfacing.  Chemical peels can remove several layers of skin and sun damage, replacing it with new skin.  Microneedling involves the use of tiny needles that penetrate the lower layer of skin, the dermis.  The dermis is where collagen and elastin are produced, and the trauma from the microneedles induces the skin cells to produce more.  When the wrinkles and sun damage are severe, laser resurfacing may be needed.  Fractional CO2 laser resurfacing uses tiny laser beams to not only stimulate collagen and elastic production in the dermis, but to also remove the upper layers of the skin.  Each of these procedures have varying levels of discomfort and downtime, and a free consultation can help you decide on the best procedure for your needs.…

What Botox will do to your expression?

What is Botox? Botox has been around for cosmetic purposes more than 10 years.  It has safely been injected in millions of patients.  Botox is a purified protein that used to relax the muscles of facial expression that cause wrinkles.  Repeated activity of facial muscles will eventually causes lines and wrinkles in the skin.  Botox injected into these muscles causes these wrinkles to not only smooth out, but also to prevent them.  But what will Botox do to your facial expression? What Botox will do to your facial expression. The goal of Botox injections is not to completely freeze the muscles, but to relax them enough to smooth and prevent wrinkles.  So what Botox does to your facial expression is usually good. Relaxing frown lines can make you look less angry and tired.  Botox forehead is used to perform a brow lift.  Botox crow’s-feet can make your eyes look wider.  Botox lips can make your lips poutier.  All of these effects from Botox will make you look younger and more refreshed but still have normal facial expressions. How to get rid of facial wrinkles. The first thing to remember is those wrinkles didn’t form overnight and will not go away overnight either.  Getting rid of wrinkles takes a multi-faceted approach coupled with continued maintenance.  There is no miracle cure that will take care of every wrinkle.  A general understanding of why wrinkles form is needed to properly treat them. What is Collagen? The building block of skin is collagen.  When we are younger we make plenty of collagen and its all in good shape.  As we age, mostly due to sun exposure, our collagen production slows down even to the point of losing collagen.  This loss of collagen causes wrinkles, sagging and generally older looking skin.  The key to treating wrinkles is to identify the factors contributing to loss of collagen and restarting collagen production.A great way to look at getting rid of wrinkles is to imagine the face as a canvas.  The canvas itself is your facial skin.  Things like Botox and Juvederm are the paint.   With a fresh, new canvas, an artist can paint a beautiful picture.  However, starting with a damaged canvas, even the best artist will have difficulty painting on it.  So trying to use Botox and Juvederm to fix wrinkles on sun damaged skin will also not be very effective.  The best first step is to rebuild the canvas, the facial skin, so that Botox and Juvederm can do what they are designed to do. Collagen Stimulation. So how do we restart collagen production?  The cells that produce collagen are in the deep layer of the skin, the dermis.  We must reach them and stimulate them to produce new collagen. There are several ways to do this.  Laser resurfacing is the gold standard and involves a Fractional CO2 laser.  The laser beam penetrates the dermis and directly stimulates the collagen cells.  The heat generated by the laser also causes the upper sun-damaged layer of skin to peel while causing the collagen already present to tighten.  Laser resurfacing produces great results but does have 7 days of recovery.An easier, less aggressive method to stimulate collagen production is microneedling.  Microneedling uses multiple small needles to penetrate into the dermis and directly stimulate the collagen cells.  Performed in-office under local anesthesia and has much less recovery than laser resurfacing.  It is repeated every 6 weeks to get maximum collagen stimulation. Skin Care. Skin care products are also key to stimulate collagen production and keeping it going.  No reason to spend money on expensive procedures and not protect you investment with the right skin care products.  Retin A and other retinols directly stimulates collagen cells to make collagen.  If one product is used, it is Retin a for its anti-aging properties.  There are many other growth factors, peptides and antioxidants that are able to promote and prolong collagen production. Botox and Juvederm. So we have now rebuilt the canvas with collagen stimulation procedures and we are maintaining it with skin care products.  This is a great time to add Botox and Juvederm to our regimen. Fraxel Laser

Laser ResurfacingWhat is Fraxel Laser?Fraxel Laser is a form of laser resurfacing and skin tightening using a fractional CO2 laser.   The laser beam is split into thousand of little dots that are used to perforate the skin (fractional laser).  This causes the upper layer of skin where discoloration, roughness and fine lines are present,  to peel off in 3-4 days.  This leaves a shiny, new fresh upper layer of skin that alone will take years off the face.  The fraxel laser also introduces a great deal of heat into the lower layer of skin where collagen is made.  Collagen is the building block of skin.  The heat stimulates the skin cells  to make new collagen and leads to skin tightening starting 3-4 weeks after the procedure.  This skin tightening will go for months and can be aided by the use of retinoids and growth factors.Is laser resurfacing safe? Does it hurt? By splitting the laser beam into scattered dots, the old, untreated skin is maintained between the dots.  This old skin acts as a natural band aid that stays on until the new skin is ready to emerge.  As a result, the risks of infection and scarring is greatly reduced.  It is an intense procedure and we apply topical anesthetic cream 45 minutes before the procedure.  After the procedure your skin is cooled with cold, wet soaks.  You will continue a specific skin care regimen at home.What is the downtime?There will be several days of redness and swelling until the skin peels.  After the skin has peeled, the skin will be pink for up to a week .   It will be necessary to keep the skin moist with the products we provide as it heals.  We will also prescribe several oral medications including pain medication to help with your recovery.  Sun exposure must be strictly avoided after the procedure.Is fraxel laser the best choice for me?Bang for your buck, laser skin resurfacing is the best treatment to remove sun damage and tighten skin.  There are other procedures with less downtime like the Whisper laser peel, chemical peels and photofacials.  A free consultation is required to determine which procedure is best for you.What else can Fraxel laser treat?Acne scars are very troubling for the patient and difficult to treat.  They consist of damage in the deep, dermal layer of the skin.  Surface treatments are ineffective.  Laser resurfacing offers the ability to remove the upper layers of skin and introduce heat into the dermal layer.  The heat stimulates skin cells in the dermis to make collagen and remodel the collagen present.   This can lead to softening of the acne scars and multiple treatments are required.…

Acne Scars Removal

How to Get Rid of Acne Scars. As teens, a lot of us dealt with acne. Some still deal with acne in the form of acne scars, which can cause extreme frustration. Difficult to treat, acne scars effect several layers of skin and often require extreme actions to address. Unfortunately, all treatments come with the caveat that while we can often minimize the appearance of scars, it is extremely difficult to eliminate acne scars altogether, so combination treatments may be applied to get the best results possible.Types of Acne Scars Three main subtypes of acne scars have been described to include rolling, ice-pick and box car.  Their appearance is self-explanatory and they can be found in various combinations.  Treatment for each type may require specific procedures for best results.Microneelding for Acne Scars Our newest tool in the fight against acne scars is Microneedling. A microneedle consists of an automated pen with tiny needles. As the provider rolls the pen across a patient’s face, it makes tiny perforations in the surface of the skin. Because the body recognizes that the skin is damaged, it immediately sends repair functions to the area to build up the skin again. This repair-damage-repair sequence, when repeated 3-5 times, can stimulate collagen and cause the skin to repair itself starting from within, improving the appearance of scarring. In addition, a serum from your blood (PRP) is applied to the just-rolled face which is packed with proteins that are vital building blocks in the skin. As it sinks into the deeper layers of the skin, now accessible because of the perforations of the roller, the serum aids in the healing process that happens after rolling is complete. In comparison to some of the other treatments we will discuss, microneedling is on the very low side of the pain scale with some patients describing it as comfortable. It also has very little to no downtime; the skin will look slightly red after the treatment and patients may be advised not to apply makeup or other lotions to the skin for several hours after but the areas will look normal within hours of the treatment.Dermal Fillers Acne scars typically leave noticeable divots in the skin.  One very effective acne scars removal method, is to inject dermal fillers into them.  A dermal filler like Juvederm is carefully injected into and under the skin to lift and fill them.  The effect from the dermal filler may be permanent or sometimes needs to be repeated.  Dermal fillers may be combined with subcision, where a needle or small blade is used to cut the scar tissue holding the acne scar open.Laser Resurfacing Our final tool in the fight against acne scars is CO2 Fractional Laser Resurfacing, which is associated with the most downtime, discomfort and efficacy of any of our acne scar treatments. In a 45 – 60 minute treatment, a fractional laser is applied to the skin, causing perforations is a DOT pattern across the entire affected area. This treatment is the most effective in treating scarring because it reaches the deepest layers of skin to initiate the healing process. With 7 – 10 days of downtime, laser resurfacing presents the most significant downtime but in many cases, only requires one treatment to see significant results in the reduction of acne scars. Fractional Laser CO2

What is Fractional Laser CO2? The  gold standard of laser resurfacing, has been the CO2 laser for years.  Offering great results, the old CO2 laser required general anesthesia and had an extended recovery.  The problem with the procedure was that the laser would remove all the surface skin.  This was very effective at erasing signs of aging, but left the patient with an open burn wound to deal with.  This led to more than 2 weeks of difficult downtime.  The advance of the Fractional laser CO2 was to not remove all the skin.  A fractional pattern to the laser beam allowed only some of the skin to be removed while leaving untreated areas between the treatment zones.  The same amount of energy as the old CO2 was delivered, but the old skin remained intact.  This old skin acts as a natural band-aid to protect the new skin until it is ready.  There are no open wounds and recovery is much easier and quicker.The old skin containing the sun damage peels in 3-4 days, and a fresh, pink, shiny new skin is reveled.  There will be some redness for another 3-4 days that gradually subsides.  The exciting thing is that this is just the beginning of the results.  The resurfacing does cause a dramatic change in appearance alone.  However we also introduce a great deal of heat to the lower layer of skin where the collagen is made.  Around 3-4 weeks after the procedure, new collagen will begin to be made and lead to skin tightening.  So the fractional Laser CO2 offers both resurfacing and skin tightening in one procedure, with much less downtime.More about Fractional CO2 Laser here.…
