What is Smart Liposuction?

Ten years ago Smartlipo Laser Liposuction was introduced.  It promised to be less invasive and to deliver better results.  The heat from the laser was used to melt fat and tighten skin.  Since then, we have learned a lot about laser liposuction and liposuction in general.  We now realize that Smartlipo is just one tool that is in a liposuction surgeon’s tool box.  Smart Liposuction now consists of many components including tumescent anesthesia, microcannula liposuction, power assisted liposuction, aggressive liposuction, laser liposuction, compression and open drainage.

Tumescent Anesthesia

The most important innovation in smart liposuction is the use of tumescent anesthesia.  Tumescent anesthesia involves the use of a local anesthesia solution consisting of saline, lidocaine and epinephrine.  After making several small incisions, this solution is infused into the fat layer between the skin and muscle.  The solution provides anesthesia, controls any bleeding, lifts the fat and skin off the muscle layer and the liquid allows the solid fat to be suctioned away.  Much of the tumescent anesthesia will be removed during the procedure, but enough will remain to allow continued anesthesia 8-12 hours after the procedure.  The remaining solution will be allowed to drain over the next several days and/or be harmlessly absorbed by the body.

Considered by many as small part of smart liposuction, tumescent anesthesia is actually the most important part of the procedure.  Tumescent anesthesia allows the patient to be awake during the procedure avoiding general anesthesia.  General anesthesia is far riskier than liposuction, and by avoiding it we make the procedure much safer and less expensive.  Since the patient is awake, we can move them around to obtain even better body sculpting results.  Plus at the end of the procedure, the patient is back on their feet immediately thereby avoiding most postoperative complications.

Microcannula Liposuction

The actual tools we use for liposuction have also improved.  The cannulas are the hollow tubes that we move through the fat layer to remove fat cells.  We now use microcannulas which are smaller in diameter and allow us to do much finer work.  These microcannulas have allowed us to perform more precise liposuction while minimizing trauma and pain as well as the risk of liposuction irregularities.

Power Assisted Liposuction

No matter how you do it, liposuction can be hard work.  The surgeon may need to move the cannulas thousands of times through the fat during a procedure.  The power assist is a handheld machine that assists the surgeon during the procedure.  This allows the surgeon to remove more fat from the patient with less trauma and effort.

Aggressive Liposuction

For many years, we used to be very concerned with performing liposuction to aggressively.  With the advent of tumescent anesthesia and microcannulas, experienced surgeons have learned how to deliver even better liposuction results.  Aggressive liposuction involves not only removing the deeper layers of fat but also some of the superficial fat.  By removing this fat right under the skin, we maximize skin tightening results in many patients.

Laser Liposuction

We have now learned to use Smartlipo laser liposuction more effectively.  It is excellent for melting small amounts of fat and causing skin tightening.  It is ideal for chin liposuction.  We also use it in cases where there is a great deal of loose skin that we may not be able to tighten with aggressive liposuction alone.  The extra heat delivered by the laser gives us an extra amount of skin tightening.


After liposuction, the main thing that needs to be dealt with is swelling.  All that empty space where the fat was is now filled by the body with fluid.  We immediately place the patient in a compression garment that closes this space and pushes the fluid from it.  Over the next several weeks to months after the procedure, the body will gradually remove the swelling.  We also help this process with massage and exercise.

Open Drainage

At the end of the procedure, we will leave all the incisions we made open.  We do this to allow your body to drain any remaining tumescent fluid and fat from the treated areas.  There are no tubes, the fluid drains by gravity and the more you move the more you drain.  The more you drain, the faster you get better!  The draining fluid also removes any bacteria, thereby also preventing infections.  The incisions will naturally close on their own after 1-2 days.
