Male Gynecomastia is a benign enlargement of the male breast resulting from growth of the glandular tissue of the breast. Men are born with breast tissue similar to women. The difference is that women produce the estrogen necessary for the breast tissue to develop. During puberty or with the use of body building supplements, men can get exposed to higher levels of estrogen. This causes the growth of their breast tissue which is known as gynecomastia. Gynecomastia is defined clinically by presence of a rubbery mass that is felt under the nipples. Although gynecomastia is usually bilateral, it can also be unilateral (one-sided). Unfortunately, once the breast tissue has grown, it rarely goes away as an adult. The only real cure for gynecomastia is vaser liposuction and/or surgical excision, or male breast reduction. The breast tissue is very firm unlike the surrounding fatty tissue. It is possible to remove the breast tissue with vaser liposuction alone, but sometimes a direct approach is also necessary.
Also known as moobs, the condition known as pseudogynecomastia, or lipomastia, is characterized by fat deposition without breast tissue growth. It is usually due to obesity and stretched out skin. Unlike true gynecomastia, pseudogynecomastia can always be treated with liposuction alone.
Gynecomastia is caused by an imbalance of sex hormones. Breast tissue is stimulated by estrogen (female) and inhibited by testosterone (male). When higher levels of estrogen are present or the tissue is more sensitive, breast tissue will grow. This can occur during several periods of life including puberty, with certain health conditions, and due to the use of several drugs and supplements. A common cause is the use of anabolic steroids and testosterone for body building, which can then be converted in the body to estrogen.
A short course of a drug called tamoxifen (brand name: Nolvadex) or raloxifene (brand name: Evista) may be recommended in adolescents or men who have had gynecomastia for short periods of time. However, these drugs are not effective if the gynecomastia is not tender and if it persists more than one year. In these cases, gynecomastia surgery is best treatment.
Gynecomastia that occurs during adolescence usually resolves on its own. It may also resolve with the discontinuation of contributing medications and the treatment of underlying health conditions. If it persists, the most effective method is complete removal of the breast tissue by vaser liposuction and/or gynecomastia surgery or male breast reduction. Under local anesthesia, small incisions are made around the chest to allow for vaser liposuction. Many times, the breast tissue can be removed with vaser liposuction alone. If necessary, a direct approach may also be necessary. A curved incision is made in the areola (which hides scars), and the breast tissue is carefully removed from the body. The procedure is called a subcutaneous mastectomy or male breast reduction.. The wound is then closed and a chest binder must be worn for several weeks. In the case of pseudogynecomastia, liposuction alone can be used to remove the fat under local anesthesia. Although rare, men can get breast cancer. Any tissue removed from the body should be sent to a pathologist for definitive tissue testing. (You will be given the option to do this at an additional fee or through insurance). An initial work up by a primary care provider utilizing a mammogram may also be required.
There is slight discomfort with the instillation of the local anesthesia, and after that it is painless. The anesthesia will last for hours and there will be some postoperative soreness that can be controlled with pain medication. You will be awake during the entire procedure so there is no long recovery and you go home numb. Since there is no risk of general anesthesia, the procedure is very safe. General anesthesia itself is much more riskier than gynecomastia surgery, so by using local anesthesia the procedure becomes tremendously safer.
We try to remove all the breast tissue during the procedure. As a result, the gynecomastia should never return. Pseudogynecomastia should also never return as long as weight is maintained. However, significant weight gain will negatively affect the results and resumption of drugs that cause gynecomastia can cause any residual tissue to grow.

A small incision in the areola of the nipple may be necessary, and small liposuction incisions will also be required. After the surgery there may be some stitches that need to be removed in one week. You cannot do any heavy lifting with your upper body for at least 2 weeks. You will be also be required to wear a chest compression vest for at least 3-4 weeks after surgery.
