What is DOT Therapy Fractional CO2 Resurfacing?
DOT Therapy (Dermal Optical Thermolysis) is the ideal micro-ablative approach to skin rejuvenation. It creates thousands of microscopic perforations, which induce immediate skin tightening and stimulate new collagen growth, improving the skin’s texture and tone to fight fine lines and deep wrinkles, and even acne scars.
DOT Therapy also leaves the skin around each of the perforations intact, which allows the epidermis to heal from the edge of these tiny holes very rapidly, significantly decreasing downtime and promoting rapid healing. As a result, SmartXide DOT offers the results of the traditional CO2 laser, but under local anesthesia with less discomfort and less downtime! SmartXide is an improvement over older Fraxel technology.
How can DOT Therapy help?
With DOT therapy, the laser beam is broken up into many small microbeams (DOTs). These are separated so that when they strike the skin’s surface, the small areas between the beams are not hit by the laser and instead, left intact. These small areas of untreated skin promote a much more rapid recovery and healing with less risk of complications. The small areas treated by the fractional microbeams, called micro treatment zones, cause sufficient laser injury to promote new collagen production and result in facial skin rejuvenation.

As you can see to the left, the initial heating of the skin will cause some immediate skin tightening due to shrinkage of collagen fibers. Three to four days after the treatment, the new upper layer of skin will be ready and the old skin will peel. This gives a fresh, new surface. At around 30 days, the new collagen production begins which will cause further tightening and softening of scars and wrinkles.
How is DOT Therapy better?
The carbon dioxide (CO2) laser has been the premier skin resurfacing tool for the past decade. Its primary advantage over peeling and other laser techniques is that it tightens the skin naturally, while stimulating increased collagen production. The results approximate that of a mini-facelift, without incisions.
CO2 ablative laser resurfacing has been the “Gold Standard” for non-surgical facial rejuvenation and removal of facial skin wrinkles and photo aging since 1995. However, due to its power and complete removal of the skin, it requires intravenous or general anesthesia. It also has several limitations including a prolonged and sometimes difficult recovery period often lasting three weeks, prolonged facial redness that can last three months, hypopigmentation or lightening of skin color, and the potential for infection which can lead to scarring.
What should I expect during treatment?
The DOT Smartxide treatment requires only local, topical anesthesia that is applied approximately 45 minutes prior to the procedure. After this period of time, there may be mild to moderate discomfort during the treatment. We do offer premedication with sedatives if requested. In most cases, the treatment itself only requires 30 minutes of your time.
Is DOT Therapy treatment safe?
DOT therapy is FDA approved. It’s much safer than many other laser resurfacing treatments. No sedation will be required, but instead a safe and topical anesthetic. Moreover, the skin around each spot will be left intact which can lead to much quicker healing. Because it works in a shallow, wide pattern, the time to heal is quick and the risk involved is minimal. Infection and scarring are extremely rare but there will be some redness and swelling for a week or two.
How many treatments will I need?
Typically, just one session is required. If problems are more severe such as acne scars, more treatments may be required. Treatments can be spaced up to two months apart.
Is DOT Therapy painful?
This procedure is slightly uncomfortable, but a topical anesthesia will be applied during treatment, and we’ll provide you with medication for three to four days after treatment to help ensure you’re not uncomfortable.
Who is a good candidate for DOT Therapy?
DOT therapy can be used to treat people of all ages and most skin types. It’s ideal to treat the signs of sun damage including pigment irregularities, wrinkles, skin laxity/sagging, and rough texture. What should I do after treatment?
Approximately three to four days after the procedure, you will peel the upper layers of skin, revealing a fresh, new surface. It is important to keep the skin clean and moist during this period. We provide you with medications during this period to promote wound healing and minimize discomfort. After this phase, there may be some residual redness but most patients can resume their normal activities. The new collagen production doesn’t begin until three to four weeks after the procedure, at which time we recommend a follow-up checkup.
